Know ye, that John M. Phillips a private of Captain R. C. Williamson Company, ( ) 79 Regiment of Ohio Inft. Vols., who was enrolled on the 15th day of August 1862 to serve 3 years or during the war is hereby discharged from the service of the United States, this 9th day of June 1865, at Louisville, Ky., by reason of G.O. No. 27 1865 Dept. of Ky.
Said John M. Phillips was born in Warren County in the State of Ohio, is 28 years of age, 5 feet 9 inches high, florid complexion, gray eyes, light hair, and by occupation when enrolled, a carpenter.
Given at Louisville, Ky., this 9th day of June 1865.
