Know ye that Jason L. Phillips a sergeant of Captain James
P. Donnelly’s Company (F) 34th Regiment of the Ohio Veteran Infantry, who was enrolled on the 4th day of January 1864 to serve three years or during the war is hereby discharged from the service of the United States, this 20th day of March, 1865 at Cumberland, Maryland, by reason of PAR Special Order No. 56 HO Quarters DW VA. Said Jason L. Phillips was born in Warren County in the State of Ohio, is 24 years of age, 5 feet 11 5/8 inches high, light complexion, blue eyes, red hair, and by occupation, when enrolled, a farmer. Given at Cumberland, Maryland, this 20th day of March, 1865
