The State of Ohio
Warren County ss

Personally appeared in open Court, being the Court of Common Pleas within and for the County aforesaid John Campbell, an applicant for naturalization, who being duly sworn declares and says; That he is sixty years of age, that he was born in the North of Ireland from whence he emigrated in the year 1791 to the United States of America, and has continued to reside there ever since, that he has been a resident of Warren County in the State of Ohio for about twenty three years past- And the said John Campbell further declares and says that during his residence aforesaid in the United States, he has been attached to the principles of the Constitution thereof, and well disposed to the good order & happiness of the same, and that it has been his intention for at least three years past, to become a citizen of the United States; and he further declares and says the he will support the constitution of the United States, and that he doth absolutely and entirely renounce and abjure all allegiance and fidelity to every foreign Prince, Potentate, State or Sovereignty whatever and in particularly to William the fourth the king of Great Britain & Ireland &c. and the said John also saith that he has never borne any hereditary title, or been of the order of Nobility in any country whatever, and further saith not.

Personally appeared in open Court T. R. Ross who being duly sworn deposeth and saith that he is well acquainted with the within named John Campbell, that he was residing within the limits and under the jurisdiction of the United Stated before the 18th day of June 1812 and has continued to reside therein ever since. That said John for at least five years preceding this time has been and continues to be a resident of the County of Warren in this State & deponent has been informed and believes that he has behaved as a man of good moral character attached to the principles of the Constitution of the United States, and well disposed to the good order & happiness of the same, and further saith not.

Personally appeared in open Court Absalom Fox who being duly sworn deposeth and saith; that he is well acquainted with the within named John Campbell, that he has for at least five years preceding this time, been and continues to be a resident of Warren County in this State, that he has behaved as a man of good moral character, attached to the principles of the Constitution of the United States, and well disposed to the good order and happiness of the same,; and further saith not.
